We offer free COVID-19 testing in Little Village and at-home tests delivered through the mail for patients with and without COVID-19 symptoms. The at-home tests are only available for Chicago residents. Would you like to receive a COVID-19 test at our Connect Chicago Little Village site or an at-home test through the mail?
At-home tests are provided through a partnership with the City of Chicago and are intended only for Chicago residents. Do you confirm that you live in Chicago?
Do you have any possible COVID-19 symptoms including cough, fever, shortness of breath, body aches/fatigue, loss of smell or taste, headache or runny nose?
Have you been directly exposed? According to the CDC, direct exposure results from being within 6 feet of someone with a confirmed positive result for more than 15 minutes.

Recommendation
At this time, we do not recommend re-testing if you have had a recent positive result. Pleas follow the CDC guidelines regarding isolation. For information on procedures following a positive result, please visit rush.edu/testresults.

These time slots are offered to help us keep track of requests. Your test kit will still be mailed to your home. You do not need to go anywhere to receive or complete your test.
Please schedule an appointment for a COVID-19 test. You will not be billed for this COVID-19 test, even if you do not have insurance. Insurance is not required to receive a COVID-19 test. Most insurance plans are reimbursing providers for the cost of providing a test, so if you do have insurance please provide that information when asked.
