Do you think you have been exposed to the COVID-19 novel coronavirus?

Do any of the following statements apply to you?

  • I’m over 40 years of age.
  • I’m pregnant.
  • I have heart or lung disease.
  • I am a health care worker or first responder.
  • I have diabetes.
  • I am immunocompromised or have cancer.
  • I live in a shelter, nursing home or dorm.
  • I am currently homeless.

Do you have any possible COVID-19 symptoms including cough, fever, shortness of breath, body aches/fatigue, loss of smell or taste, headache and runny nose?

Recommendation:

Please start an on-demand video visit for a provider consultation, 7 a.m. - 9 p.m, seven days a week.
