Take Charge of Your Health
The Take Charge of Your Health workshop meets once a week for six consecutive weeks in community-based settings, such as churches, hospitals, and senior or community centers. It provides individuals age 60 and older who have a chronic condition(s) with skills and information to better self-manage their conditions. Approximately 400 individuals in the City of Chicago and suburban Cook County will be recruited to participate. At Rush University Medical Center we expect to recruit approximately 24 to participate.
You may be eligible to participate in this study if you:

Come to every session on time (the sessions will be held once a week for six weeks in a row for about two to two and one-half hours)

Make and complete an action plan each week

Give any new activities a try for at least two weeks before deciding what works best for you

Maintain confidentiality (what is said during the workshop must not leave the workshop)
