Access to your health record
Rush offers multiple ways to access your health record, connect with providers and learn more about your care. When you create a health record account you can do the following:
- Access medical history and immunization records
- View test results
- Find out detailed information about the medications your doctor prescribes and the medications you're currently taking
- Send messages to your care team
- Pay your medical bills
- Schedule appointments
MyChart
If you're accessing your health record on your computer, use MyChart. To create an account on MyChart, visit the MyChart registration page. Use the access code provided at the time of your appointment or enter your information to create an account.
Both outpatients and inpatients can also access MyChart by filling out an access request form. If you include your email address on the form, Rush will email your activation code. Fill out the form and then email or fax the form to the Health Information Management Office:
Email: mychartrequest@rush.edu
Fax: (312) 942-5549

The My Rush app contains the same information as your MyChart account, plus additional features such as video visits, driving directions, a symptom checker and more.
Go to the App Store (for iOS) or Google Play (for Android) on your mobile device to download the app.
