Community-Based Practices – Providing West and South Side Youth Accessible, Affirming Care
COVID-19 Updates
For additional up-to-date information about COVID-19, please visit the Rush (COVID-19) Information and and Resources page.
The Rush Community-Based Practices comprises the five School-Based Health Centers (SBHCs) located inside of public schools on Chicago's West and South Side and the Adolescent Family Center (AFC), a health center for young people on the Medical Center campus. These six clinics increase access to vital preventive, primary and mental health services that keep young people healthy and on track for successful adulthood.
Rush School-Based Health Centers
For over 20 years, Rush University Medical Center has provided vital and necessary health care services through school-based health centers in Chicago public schools located in underserved neighborhoods. Nurse practitioners (NPs), licensed clinical social workers, registered nurses and physicians from the Medical Center provide a full range of clinic services, including physicals and immunizations, primary care, treatment of injuries and mental health services, regardless of income or ability to pay.
Our practices aim to keep youth physically and mentally healthy, on track academically and positioned for successful transition to leading healthy, productive adult lives. With immunization rates improving, emergency room visits decreasing, the delivery of healthier babies, increased graduation rates and the implementation of healthy behaviors that help prevent chronic diseases, SBHCs are making, and continue to make, a real difference.
Rush currently operates five SBHCs:
- Rush Health Center at Simpson Academy
- Rush Health Center at Orr Academy High School/KIPP One
- Rush Health Center at Richard T. Crane Medical Preparatory High School
- Rush Health Center at Dunbar Vocational Career Academy High School
- Rush Health Center at Wendell Phillips Academy High School

Minor consent forms available in both English and Spanish.
Parent or guardian consent is required in order for persons under the age of 18 to receive medical services. Parental/guardian consent forms, available in both English and Spanish.
